Zika Disease Reporting Numbers
REPORT suspected or confirmed cases of dengue, chikungunya, or Zika virus disease to the Public Health Acute Communicable Disease Unit (ACD) by faxing a Confidential Morbidity Report form to (510) 273-3744.
FAX the ACPHD Zika Virus Testing Request Form to Public Health at (510) 273-3744 to obtain approval prior to sending specimens.
