Cross-Agency Programs

Alameda County Health strives to interact, understand, and promote accessible, responsive and effective services for the people and communities of Alameda County. We must constantly evaluate and initiate new programs for the changing needs of all residents.

Cross-Agency Programs

Housing & Homelessness Services

Implements expanded homeless services and supports and leads the development of a strategic framework to address homelessness. The Office is building a robust, integrated, and coordinated system of homelessness and housing services in Alameda County to better serve our most vulnerable residents.

 

Emergency Medical Services

The Alameda County Emergency Medical Services Agency (ALCO EMS) is the Agency that plans, implements, and oversees all Emergency Medical Services (EMS) activities conducted in Alameda County.

Health Program of Alameda County (HealthPAC)

Provides affordable health care to uninsured people living in Alameda County.

 

Health Insurance Enrollment Assistant

The Health Insurance Enrollment Assistance department pre-screens Alameda County residents on the phone and provides information and referrals for application assistance for the following health insurance programs: Medi-Cal, CalFresh, Cash Aid, Healthy Families, and Kaiser Child Health Plan. Health Insurance Technicians can schedule in-person enrollment assistance appointments to complete new and renewal applications. Enrollment assistance is available by appointment only. Regardless of immigration status, applicants may be eligible for a program.

The Essential Health Care Service Initiative.

 

Center for Healthy Schools and Communities

Because health and academic success are deeply connected, our health and education systems should be too. Integrating health into schools makes sense—that’s where kids and families are. The need is clear. Too many children lack access to the basic conditions and life opportunities that support wellness and school success.

CASA

(Court Appointed Special Advocate) A nationally-based organization that provides one-to-one court advocacy to abused, neglected and abandoned children who are dependents of the juvenile court.

 

MAA – TCM

The MAA Program offers a way for Local Governmental Agencies (LGAs) and Local Educational Consortia (LECs) to obtain federal reimbursement for the cost of certain administrative activities necessary for the proper and efficient administration of the Medi-Cal program. MAA activities include: Medi-Cal outreach, Facilitating the Medi-Cal application, non-emergency transportation of Medi-Cal eligible individuals to Medi-Cal covered services, Contracting for Medi-Cal services, Program planning and policy development, MAA coordination and claims administration, MAA Training, and General administration.

The TCM Program provides specialized case management services to Medi-Cal eligible individuals in a defined target population to gain access to needed medical, social, educational, and other services. TCM services include: Needs assessment; Development of an individualized Service Plan; Linkage and Consultation; Assistance with accessing services; Crisis assistance planning; and Periodic review. TCM program types are adult probation, outpatient clinics, linkages, public guardian, public health, and community.
