The Alameda County Public Health Department (ACPHD)'s mission is to work in partnership with our local communities to ensure the optimal health and well-being of all people. This includes monitoring health status and service delivery, preventing disease, mobilizing communities, conducting outreach, and promoting health policy and education. ACPHD works collaboratively to improve health at both the community and population levels. It works with respect to the diversity of our communities and with deference to past, present, and future generations.

Community Assessment, Planning, and Evaluation (CAPE)

The Community Assessment, Planning, and Evaluation (CAPE) Unit conducts data analysis and evaluation for the Alameda County Public Health Department (ACPHD) as well as for policymakers, partner organizations, and Alameda County residents.

Community Health Services

The Community Health Services Division aims to empower residents to prevent and mitigate chronic disease and be resilient and healthy at all ages. Our programs address healthy communities, systems, and environments; community health promotion and prevention; and health navigation and chronic disease supports, including care coordination and case management.

Division of Communicable Disease Control & Prevention (DCDCP)

The Communicable Disease Control & Prevention Division collaborates with the community to safeguard public health by preventing the spread of communicable diseases and managing health emergencies. They offer programs such as disease surveillance, immunization, and education.

Family Health Services

The Family Health Services Division works to improve the health and well-being of Alameda County’s diverse families with compassionate, comprehensive and collaborative services.

Office of the Public Health Director

The Office of the Public Health Director is the central office at ACPHD, responsible for setting policy and providing overall leadership to Public Health activities.

Public Health Nursing in Alameda County offers diverse health services and education to individuals and communities, aiming to enhance health equity and well-being through outreach, coordination, and advocacy.
