Alameda County Housing and Homelessness Services (H&H) works to build a robust, integrated, and coordinated system of housing and homelessness services, serving as the County’s central point of contact for homelessness strategic planning and program development. To improve health and housing outcomes for people experiencing homelessness, H&H oversees Coordinated Entry and System Access services and the Homeless Management Information System (HMIS), and collaborates with county agencies, city partners, and more than 50 service providers to deliver comprehensive crisis response and diversion, interim housing, and permanent housing services across the County’s homelessness response system.

Alameda County Health Care for the Homeless (ACHCH) is a federally-funded health center program housed in Alameda County Health. We provide direct care services and coordinate a network of county and community health clinics and organizations to increase access and improve care for people experiencing homelessness.
The Housing Services Division within H&H provides systems development and implementation for housing programs serving people experiencing homelessness throughout Alameda County.
