The Community Health Services Division aims to empower residents to prevent and mitigate chronic disease and be resilient and healthy at all ages. Our programs address healthy communities, systems, and environments; community health promotion and prevention; and health navigation and chronic disease supports, including care coordination and case management.

Our Mission

The mission of CHS is to encourage, support, and empower residents to be healthy, build capacity for self-sufficiency, and improve the health and well-being of the community. CHS focuses on neighborhood-based community strategies that address root causes of health and social inequity in public education and income and economic development.

Key strategies that CHS currently uses include:

  • Promoting healthy choices through policy development, community engagement, education, and information dissemination.
  • Protecting the health and well-being of residents with appropriate interventions and health policies based on state-of-the-art knowledge.
  • Ensuring access to quality health and social services through collaboration with individuals, families, institutions, and available resources in the community.
  • Providing culturally and ethnically sensitive services to the community.
  • Engaging in ongoing planning and evaluation in partnership with the community.

Nutrition Services

We promote and support healthy eating and physical activity through committed community partnerships to reduce chronic disease and improve long-term health.

Office of Dental Health

The Office of Dental Health supports efforts to improve the oral health of Alameda County residents by partnering with the community to assess oral health status and resources and to assure access to community-based services and oral health education. We actively engage in policy development that incorporates evidence-based dental disease prevention and promotes oral health equity.

Other Community Health Services Programs

The Community Health Services Division also contains the following programs: Alcohol and Other Drug Prevention Program (AOD), Asthma Start Program Care Partners, Healthy Brain Initiative, Healthy Nail Salon Program, Older Adults Healthy Results Program, Project New Start, Recipe 4 Health, and Tobacco Control Program (TCP).

Women, Infants & Children (WIC) Supplemental Nutrition Program

WIC is a national supplemental food and nutrition program for growing families that provides benefits for healthy foods, nutrition education, breastfeeding support, and other community services.
