Housing Community Supports

Alameda County Health’s Housing and Homeless Services (H&H) partners with 24 community-based housing and homeless service providers to administer Housing Community Supports. Funded through California Advancing and Innovating Medi-Cal (CalAIM), Housing Community Supports provides services to over 3,000 of Alameda County’s most vulnerable homeless and formerly homeless households. Services include:

  • Housing Transition Navigation services, to assist consumers with obtaining housing;
  • Housing Deposits, to assist with one-time services and modifications necessary to enable a person to establish a basic household; and
  • Housing Tenancy and Sustaining Services, to assist in maintaining safe and stable tenancy once housing is secured.

 

Home Together Move-In Assistance Fund

Alameda County’s Housing and Homelessness Services Office, in partnership with Abode Services, is pleased to announce the launch of the new Home Together Move-In Assistance Fund. This new fund will replace the existing Home Stretch Housing Assistance Fund and Housing Community Supports Housing Deposits programs. Starting July 1, providers assisting Alameda County residents and families experiencing and/or exiting homelessness will now be able to request funding for eligible move in-related costs through a portal operated by Abode Services.
