As part of Alameda County Health, the Public Health Department (ACPHD)’s mission is to work in partnership with our local communities to ensure the optimal health and well-being of all people. This includes monitoring health status and service delivery, preventing disease, mobilizing communities, conducting outreach, and promoting health policy and education. ACPHD works collaboratively to improve health at both the community and population levels. It works with respect to the diversity of our communities and with deference to past, present, and future generations.
ACPHD staff is committed to ensuring everyone in Alameda County leads a healthy, fulfilling, and productive life regardless of who you are, where you live, your financial status, or your racial and ethnic background.
To identify the needs of residents, ACPHD conducts A Community Health Assessment (CHA) and develops a Community Health Improvement Plan (CHIP). A CHA is a collaborative process of gathering information to educate and mobilize communities, set priorities, secure resources, and plan actions to improve regional health. It sets the foundation for developing a CHIP, which outlines an action plan to address key areas based on the health, safety, and well-being needs identified by the CHA. The CHA helps health officials and community members work together to set priorities, allocate resources, and implement projects and programs to enhance community health and wellness over the long term.
Alameda County Public Health Department’s 2025-2028 Community Health Assessment report will be published and disseminated in March 2026, and the Community Health Improvement Plan planning process will start in the spring of 2026.
