Alameda County Health, Behavioral Health Department (ACBHD) Adult & Older Adult System of Care serves adults between the ages of 18 and 59 years. We also help older adults aged 60+ whose serious mental illness or co-occurring illnesses seriously impact the activities of daily living.
We serve individuals who are Medi-Cal recipients, Medi-Cal eligible, Medi-Cal/Medicare recipients (Medi-Medi), HealthPAC recipients, and uninsured individuals.
Please note that we do not serve individuals with commercial/private Medicare plans or persons insured by commercial/private insurance plans.
Services that are available to Adults and Older Adults include but are not limited to the following:
- Assessment
- Therapy
- Psychiatric medication support, management, and consultation
- Urgent medication support
- Case Management/Intensive Case Management for care coordination and support
- Acute and sub-acute inpatient hospitalization
- Housing support services like supported residential living
- Employment
- Outreach and Engagement
- Community Integration Services for persons transitioning from the criminal justice system
- Prevention and Early Intervention Services for Underserved English Limited Proficiency Services
- Specialty Care Coordination Programs including CARE Court and Community Conservatorship
