About Home Together

Home Together serves as Alameda County’s unified approach for guiding homelessness policy and funding decisions. It includes the Home Together Strategic Plan and the Home Together Fund, supported by the 2020 Measure W half-cent sales tax. The Home Together work aligns strategic priorities with funding decisions, linking long-term system planning with accountable, equity-centered investments.

Draft Home Together 2030 Plan

Grounded in updated system modeling, racial equity analysis, countywide data, and community input, Home Together 2030 provides a shared roadmap for reducing homelessness, advancing equity, and strengthening the systems and partnerships needed to help more people move from crisis to safe, stable housing.

This draft plan reflects extensive planning, analysis, and community input, but it is not final. H&H collected input on the draft plan during a 45-day public review and comment period that closed on October 1st, and is now finalizing the plan to reflect this feedback. We anticipate that the final Home Together 2030 Plan will be released by the end of the calendar year.

Home Together 2030 Strategic Plan Community Meetings

These meetings were hosted online in August and September 2026 by each Alameda County Board of Supervisors member. Attendees heard from community members and shared their feedback on strategies for engaging people experiencing homelessness, removing barriers to assistance, and creating clear, navigable paths to shelter, housing, and other support.
