Message from the Director

Ten years ago, SHIE began with a clear idea: Alameda County could better serve residents by connecting care across health, behavioral health, and social services. What began as a vision, supported by Whole Person Care funding and strong community partnerships, has become trusted infrastructure for whole-person care.

Today, SHIE is entering its next phase. The work ahead is not only about sustaining what has been built. It is about increasing impact, strengthening accountability, and using data more effectively to improve health and equity across AC Health.

Our path forward is grounded in four strategic priorities.

First, deepen trust and accountability across our partner network. The SHIE exists to be a force multiplier for county agencies, health plans, providers, and community-based organizations continue to work together in service of shared goals. We are strengthening governance, transparency, and shared expectations so partners can rely on the integrity of the data, the clarity of the process, and the value of participation.

Second, strengthen SHIE as the connective infrastructure for interoperable, whole-person care. Effective care coordination depends on timely, reliable information. We are improving data quality, standardization, and interoperability so clinical, behavioral health, and social needs information can move more effectively across the health network. In doing so, we are reinforcing a broader data-sharing ecosystem that supports better coordination, more timely decisions, and stronger service delivery across AC Health.

Third, equip and empower the workforce with better tools and intelligence. SHIE creates the greatest value when care coordinators, case managers, outreach workers, and other frontline teams have timely, actionable information and the ability to use it well. We are investing in user-centered design, stronger system integration, and responsible AI-enabled capabilities that can reduce administrative burden, surface useful insights, and give staff more time to focus on the people and families they serve.

Fourth, measure impact with equity at the center. Data should lead to better outcomes. We are sharpening our focus on measurable improvements in service delivery, care transitions, duplication reduction, and community health outcomes. At the same time, we are using data to identify disparities, reveal gaps, and guide action so progress reaches the communities that have historically faced the greatest barriers.

SHIE is no longer a pilot or an emerging concept. It is an essential part of coordinated care in Alameda County and of a broader data-sharing ecosystem that supports better service delivery across AC Health. As healthcare continues to evolve through interoperability, shared data, and the responsible use of AI, SHIE can help connect systems, support better decisions, and improve how care is delivered. The goal remains the same as it was at the start: to place the person at the center and build a more coordinated, equitable system of care.
