How to Apply for CCS Case Management Services
- Anyone can refer a child or youth to CCS and help them apply. Often, a doctor will identify that a child or youth may have a CCS-eligible medical condition.
- An application for CCS services must be filled out and signed by a youth 18 or over or a child’s parent, legal guardian, or family member. CCS staff will review the application.
- CCS will request reports from the child or youth’s doctor about the condition, injury and/or disability that may make them eligible for CCS. The report will be reviewed by the CCS nurse case manager.
- Application forms should be sent to CCS via fax at (510) 273-3884.

Getting Approved for CCS
- CCS will review the application and medical reports to determine if the child or youth can get CCS. If the client meets eligibility, the child or youth will be approved.
- If the client is not eligible, the application is denied.
- A letter will be mailed to the address on the application to inform of the approval or denial.
- If CCS is denied, the youth or child’s parent/guardian may file an appeal. The Dept of Health Care Services has information on how to file an appeal.
- If CCS is approved, the child or youth will become a CCS client and will be able to get services paid for by CCS. Each client will have a CCS nurse case manager who helps them get the right care at the right place.
Getting Services Authorized (or Approved)
- Each time the child or youth needs to see a new doctor or get new medication or equipment, a Service Authorization Request form (SAR) needs to be completed.
- The medical provider will fill out the SAR and send it to CCS. You will be notified by mail if the services have been approved. You can contact CCS to find out the status of a SAR.
- Once approved, the family/client can schedule an appointment for the care that was authorized.

Payment for Services
When a SAR is approved, CCS pays for services related to a child’s or youth’s medical condition. CCS is not a health insurance program. However, CCS covers services that are not otherwise covered by Medi-Cal or private insurance.

How do I apply for Medical Therapy services?
Just like the CCS Case Management program, anyone can refer a child or youth to the Medical Therapy Program.
- A medical provider can refer a child or youth by submitting reports that describe their patients’ medical eligibility and need for PT and/or OT. When the referral comes from a medical provider, CCS will send the young adult or family an application to complete.
- Young adults and families can also submit an application. The application must be filled out and signed by a youth 18 or over or a child’s parent, legal guardian, or family member. CCS staff will review the application.
- CCS will request reports from the child or youth’s doctor about the condition, injury and/or disability that may make them eligible for CCS. The reports will be reviewed by CCS to see if the child or youth is eligible.
