If you need a birth or death certificate, please visit the Alameda County Recorder’s website. Contact our Vital Registration staff if you need a copy of a home birth certificate.

We do not collect any health records from doctor’s offices. For health records including vaccinations, please contact your health provider. For more recent vaccinations, check the California Department of Public Health website.

If you need environmental records or data about a particular address in Alameda County, there is a form on the Alameda County Department of Environmental Health website.

Before placing your request, please check our Data and Dashboard section for the specific data you are looking for. Much of our data is also on Healthy Alameda County, a website for community health information and population data.

We do not release individual-level data. Also, we have strict usage agreements for datasets and so some data may be masked for confidentiality.

Normal turnaround time for completing data requests is two weeks.

Data will be sent via email unless otherwise indicated by requestor.
