Community Health Assessment

The Community Health Assessment (CHA) takes an overarching look at the health of Alameda County residents by studying a combination of the social determinants of health and specific health outcomes of individuals, neighborhoods, and populations. 

The Community Health Assessment is completed every three years as an important tool for informing the community about Alameda County residents’ health. It also serves as a basis for: 

  • Priority setting
  • Policy changes
  • Coordination of community resources
  • Identifying disparities among different subpopulations and factors that contribute to them 
  • Supporting efforts to achieve health equity  

The assessment is based on 35 focus groups in seven different languages completed in partnership with 21 organizations, dozens of key stakeholder interviews, and quantitative analysis of 12+ major reports, completed from summer 2024-early spring 2025.

Upon completing this process, the Alameda County Public Health Department shared draft Community Health Assessment findings with 115+ community members at five community forums to hear their feedback and input. This feedback and input were included and shaped the final Community Health Assessment report. 

The CHA identified and prioritized the following key issues: 

  • Social Determinants of Health 
  • Economic stability
  • Health care access and quality 
  • Housing and homelessness  
  • Peaceful communities 
  • Racism/discrimination  
  • Built environment
  • Climate/natural environment
  • Behavioral Health 
  • Chronic Conditions 
  • Communicable Diseases  

The finalized Community Health Assessment report will be published and disseminated in March 2026. 

Community Health Improvement Plan

The Community Health Improvement Plan (CHIP) is in development and will identify ways to address the health disparities outlined in the CHA. The plan will have actionable steps for Alameda County to take to meaningfully improve health outcomes for our community.  

Project Timeline

The first phase of the project began in the fall of 2025. The second phase will launch in the spring of 2026. See timeline below:
