California Children’s Services

California Children’s Services (CCS) is a state public health program for children and youth with certain diseases or health problems. Through this program, children and youth birth to 21 years old can get the health care and services they need.

Case Management Program

Each CCS client has a nurse case manager who helps them get medical care and supplies.
CCS also has social workers who work on financial and residential eligibility. They also help connect families with mental health and other support.

Medical Therapy Program

The CCS Medical Therapy Program (MTP) provides occupational and physical therapy to children with certain physical medical conditions.
Clients get therapy at five Medical Therapy Units located in schools in different parts of Alameda County.

Enhanced Care Management

Eligible CCS clients can get a Lead Care Manager to help with life and health needs beyond the CCS condition. They can help connect clients to care and community resources for food, education support, legal help and more.

Transition Support

At their 21st birthday, young adults can no longer get CCS health services and coverage. The CCS Transition Specialist can help youth and young adults get ready for adulthood.

About CCS

CCS assists children and youth who meet these four requirements:

  1. Birth to Age 21
  2. Live in Alameda County
  3. Have a chronic, disabling, or life-threatening medical condition that is a CCS eligible medical condition
  4. Income
  • Full-scope or Share-Of-Cost Medi-Cal

or

or

  • Families with AGI over $40,000 may be eligible if their out of pocket costs for the eligible  condition are expected to exceed 20% of AGI

Children and youth can get Medical Therapy Program services no matter what their family income is.
