  1. Confirm required vaccines by grade
    Use the official California requirements for TK–12 (CDPH).
  2. Review the student’s documentation
    Check vaccine doses, dates, and exemptions.

  4. Document and report

If the record is incomplete or missing doses
Use California’s rules for conditional admission, follow-up timelines, and exclusion. Schools should document what was received and what is still needed.

Medical Exemptions

Only medical exemptions for TK–12 and child care issued electronically through the California Immunization Registry – Medical Exemption (CAIR-ME) system by a physician licensed in California are considered valid.

  • Schools should rely on valid documentation and follow state guidance for verification and recordkeeping.
