This program consists of a team of knowledgeable and trained staff that will help residents of Alameda County navigate their way towards receiving effective and essential services offered internally by ACPHD and externally by community-based organizations and health care providers.

Mission Statement

The Alameda County Health, Public Health Department (ACPHD) Front Door program serves as a referral resource and entry point to services for healthcare and community-based organizations, clinicians, county entities, and community residents seeking services internal and external to the Department. The ACPHD Front Door program will process referrals through care coordination, guidance, helpful navigation, and warm handoffs to service programs that assist individuals with changes in health care and housing services. Our referral data will help us better understand who we serve and where service gaps may exist and support generation of new policies and services that promote equity.

Program Services

We commit and strive to provide access to needed, available, and appropriate resources and services to empower residents to live healthy, safe, and productive lives.

  • Provide assistance with completing initial and renewal applications for health insurance, such as Medi-Cal, Covered California, and HealthPAC.
  • Provide assistance with completing initial and renewal applications for public assistance programs, such as CalFresh, CalWORKS, General Assistance, etc. and support with understanding and navigating the eligibility process.
  • Educate, link, and refer individuals to community resources, Medi-Cal services, Behavioral Health services, etc.
  • Provide assistance with navigating access to programs and services within local cities and Alameda County government at large.
  • Provide Information on available resources regarding communicable diseases such as COVID19, MPOX, Bird Flu, Influenza, etc.

Eligibility Criteria

  • All Alameda County residents (except for those residing in the City of Berkeley) including:
    • Low income
    • Single Adults
    • Families with Children
    • Seniors
    • Persons with Disabilities
    • Pregnant Individuals
    • Those with specific diseases such as congestive heart failure, hypertension, and diabetes
