Dental Care Coordination is a family-centered, assessment-driven, team-based activity designed to meet the needs of families with children/youth while enhancing the family’s ability to navigate the health and social service system, and access dental health services and resources.
Eligibility
- Children 0-20 years old
- Pregnant or postpartum individuals up to 12 months
- Eligible for Medi-Cal, California’s Medicaid program.
- Alameda County resident

- Scheduling dental appointments
- Oral health education
- Referrals to community resources
Care Coordination Resources
- If you work with families who are eligible for our program and your client needs assistance finding a dentist or accessing dental care, please use our Dental Care Coordination Referral Form to refer them to our team.
