Our Post-Crisis Follow-Up and Crisis Connect team is comprised of licensed clinicians, mental health specialists, and a registered nurse. We conduct follow-up services within 24-72 hours after a crisis event. The team also provides prevention and early intervention to individuals who are without housing, not receiving mental health services, and/or suffering from addiction due to a mental health condition.

These programs are 80% staffed by individuals with lived experience.

Staff may connect clients to services such as mental health, housing, community resources, or recovery programs for substance abuse. Staff partner with Healthcare for the Homeless and other providers to link consumers to ongoing mental health services.
