Case management helps facilitate client wellness and autonomy through advocacy, assessment, planning, communication, education, money management, and service facilitation.

Case Management Services

The case manager collaborates with all service providers. They link clients with the right providers and resources based on client needs and values. They do this while ensuring that the care provided is safe, effective, client-centered, timely, efficient, and equitable.

Full-Service Partnership (FSP)

We offer multidisciplinary treatment teams that are community-based and provide intensive case management services. Teams see members six or more times a month and collaborate with members using a “whatever it takes” approach. Services are primarily field-based and more intensive than Service Teams. All FSP teams are run by community-based organizations. ACBHD FSPs are based on the Dartmouth ACT fidelity model.

Service Teams

A service team provides both community-based and clinic-based mental health services that are less intensive than FSP services and have less multidisciplinary staff.

  • All teams have medication consultation, medication management, assessment, employment assistance, and other support as needed by the individual.
  • Case management teams and clinics provide services throughout the county.

Our system of care has both an FSP and a Service Team dedicated to serving older adults.
