Program Information

Nurse-Family Partnership (NFP) is a home visiting program that partners nurses and first-time moms in order to give babies the best start in life. Case management services begin in pregnancy and continue until the child reaches 2 years old. During that time, parents are provided with health education, support, and child development information so that families can create better lives for their child and themselves.

  • First-time mom
  • Less than 28 weeks gestation at enrollment
  • Low income
